We are thrilled to welcome you to our practice and are pleased you chose us to serve your periodontal needs. Our goal is to get your oral health to the highest possible standard through excellent, dedicated service. One of the best ways for our office to honor your time is to have the new patient forms before or immediately upon your arrival. Please remember to bring these with you at the time of your appointment, email, or mail the completed forms back to us as soon as possible.
Our office policy requires you to pay for the initial visit when you come in. We will submit your claim to your insurance the same day you are in our office and once your insurance makes a payment, we can either issue you a reimbursement check or we can apply the payment as a credit towards any further treatment you have in our office. Since all patients are different, Dr. Seale must complete your examination before establishing your treatment plan and the fee for care. The fee for periodontal therapy can vary considerably depending on the type of problems and the complexity of the recommended treatment. After the initial exam, you will be provided with a treatment plan that will include the services recommended and the cost of the treatment.
Payment options
We accept most insurances and will file your claim the day you are seen in our office. Our office is committed to showing you how to maximize your insurance benefits. Due to the complexities of insurance contracts, please be aware that every insurance policy varies, and while we strive to give you the best possible estimate, we cannot guarantee exact coverage. Your estimated patient portion must be paid at the time services are rendered.
Should you have concerns regarding a received statement or explanation of benefits, please do not hesitate to call or email your questions. Insurance co-payments are often confusing and we want to expand your insurance knowledge.
Patient’s portion must be paid at the time services are rendered and can be paid by any of the following payment options:
